Terri Adams and her Columbia High School FFA students visited the State Capitol on February 25, 2014. They participated in tours and workshops to learn more of its history and the legislative process. These students show the beneficial impacts of FFA and 4-H programs on our youth, building character and responsibility to prepare them for bright futures. Columbia Brazoria ISD will be offering athletic physicals for students who will be participating in athletics, cheerleading, dancing dolls, etc… for the 2014 – 2015 school year.
CBISD requires annual physicals for all students participating in athletics, cheerleading, dancing dolls, etc… In order to participate in athletics, cheerleading or dancing dolls; the physical exam must be completed before a student participates in any practice, athletic period (both in‐season and out‐of‐season) or games/matches.
COST: $25, cash preferred
WHERE: CHS Auxiliary Gym 1:00PM - 4:30PM
Physicals will performed by physicians from the Memorial Hermann Sports Medicine Group/Richmond Bone and Joint Clinic
You must have a CBISD physical form signed on all pages by you and your parents and the first 3 pages must be completely filled out prior to receiving your physical. The forms will be available in your school office or at the CHS Fieldhouse.
Physicals on forms other than the CBISD form WILL NOT BE ACCEPTED.
If a student wishes to get a physical without their parent/guardian present, they must have the CBISD physical form filled out and signed on each of the 4 pages by a parent/guardian.
· All physicals will be done at the Columbia High School auxiliary gym
· Time will be 1:00 pm – 4:30 pm (or until we finish; we will not allow anyone to enter after 4:30)
